Why would I want a blank form rather than filling it in?
Because some workplaces still collect claims on paper, and a printed form with fixed columns is far easier to process than whatever layout each person invents. It also works where the claimant has no computer access, such as field or site staff.
How many rows should I print?
Enough that a typical claim fits on one page. Around fifteen to twenty rows suits most monthly claims. If a claim regularly runs longer, print two pages rather than shrinking the rows to the point where handwriting becomes unreadable.
Should I add my own categories?
Yes, if your bookkeeping uses fixed categories. Pre-printing them removes the guesswork and stops the same cost being filed three different ways by three different people, which is what makes paper claims painful to reconcile.
